This is to remind you of what it means to be enrolled with us. The purpose of me being part of a Family Health Organization is to provide you with accessible health care in several ways: 1. I will make every attempt to see you promptly a (i.e. same day ) if you have an urgent medical problem. If I am away, Dr. Abelsohn or Dr. Lai or Dr. Carleton or Dr. Hudson will be able to see you. 2. Access to a doctor after hours. Weekday evenings from 5 to 8, and for a block of time on weekends, one of the doctors in our group is in his her own office and is available to see you for urgent medical matters. After providing medical care to you, they will fax a summary to us to put in your chart. Call our office or 647-348-0514 to find out which doctor's office is open on a given day. 3. Telephone health advice from a nurse 24 hours a day at 1-866-553-7205. The nurse sends a summary to our office the next day. 4. I am happy to discuss medical issues and test results over the phone or email, and renew prescriptions over the phone if you are enrolled with me. One of the aims of the Family Health Organization is to lessen our patients’ dependence on walk-in clinics by providing expanded access to health care. I ask you to not use walk-in clinics or see doctors outside our group unless by referral from me. (This does not restrict your access to specialists) If you do prefer to use walk-in clinics or see other general practitioners, I will take you off my list of enrolled patients. You may continue to be my patient, but without access to the above benefits. |
Family Health Organization
I am part of a Family Health Organization (FHO), a group of doctors who share on-call and cover each others’ practices. As my patient you are enrolled with the Rosedale FHO. There are 28 doctors in the FHO, including myself Dr. Abelsohn, Dr. Lai, Dr. Carleton and Dr. Hudson The doctors in our FHO have offices in the Bathurst and St. Clair area and on Yonge St. One of the goals of the FHO is to provide patients with health care services when their doctor is away. When I am away, one of the other doctors in the FHO will see you at his or her office, and will send me a note about your visit. Call 647-348-0514 to find out who is on call that day. What I ask from you is to use the services of our FHO rather than going to a walk-in clinic. If there are any difficulties with this, please let me know. If you want to continue using walk-in clinics, it’s better that you are not enrolled with our FHO. ( Obviously for major problems the Emergency Department is still the place to go).
For patients who are enrolled with me, I am happy to discuss a medical problem if it can be dealt with over the phone or by email.

Patients who are enrolled with me have access to a secure and encrypted email system through the Wychwood Family Health Centre Patient Portal. If you are a regular patient of mine and have not been invited to register with Health Myself, please let us know and we will send you an invitation.
If you need a prescription refill, call your pharmacy and they will fax me the request. Do not use e-mail for urgent matters - phone us instead. |
